Setup php form for email. How?

hi,


i have a wordpress blog and i want the following:





to make a form where people (unregistered) will choose an option from radio buttons, in another fields right their names and email, and into other fields the recipients email(s). (no mine) and so the answer they will choose (radio buttons) will be sent under their name and email to the emails they wrote before (Recipients).





and the email which the recipients will receive, will say for example





"Person a chose option 3 for you" (example)





How can i do that? (either in html or php, in order to work in an php page (e.g. Wordpress Blog).

P.S. if it is needed, i have a domain with an email in order to use it for the above "service" (form).
